All players must have all forms submitted and approved before beginning tryouts: 1. In DragonFly Max upload or fill out Dekalb Emergency Card, Dekalb athletic participaton form, cardiac arrrest form, concussion form, heat and humidity policy, health history, medical eligibility, and physical form. Use code 3U7TQB if new to DragonFly. 2. In Final Forms (accessible via infinite campus for students) upload GHSA physical form. Parents, set up an account.
